Palmetto PCA Name Tag Order Form

The name tag features the Palmetto PCA logo and your name. They are great for club meetings and events.
If you would like to order a name tag you can submit your request below.

New tags will be ordered after six or more individual orders are received.

The tags can be picked up and paid for at club meetings and most Saturdays at the weekly Trotters Restaurant breakfast.
Payment by check is prefered, if paying by check please make checks payable to PCA Palmetto Region.
